Client Acknowledgment and Waiver

1. Voluntary Participation

I am participating in this service voluntarily and of my own free will. I have read the description of the service I am booking and understand its nature.

2. Service Selected, Not Substituted

I understand the difference between Therapy, Life Coaching, Live Chart Reading, Recorded Astrology Reading, and Recorded Tarot Reading, as described in the Disclaimer. I am selecting the specific service that fits what I am looking for, and I understand that one service is not a substitute for another.

3. Therapy Acknowledgments

If I am booking Therapy, I acknowledge that this is clinical psychotherapy provided under a Licensed Clinical Social Worker license. I understand that mandated reporting laws apply to disclosures of child abuse, elder or dependent adult abuse, or imminent harm to self or others. I understand that Therapy sessions are not for psychiatric emergencies, and that in a crisis I will call 988, 911, or go to an emergency room.

I understand that I have the right to ask questions about the therapeutic process at any time, to refuse any intervention, and to end the therapeutic relationship at any time.

4. Not Medical, Not Psychiatric, Not Legal, Not Financial

I understand that no service offered through this practice is medical care, psychiatric care, legal counsel, or financial advice. For any concern outside the scope of the service I am booking, I will consult a qualified, licensed professional in that field.

5. Astrology and Tarot Are Reflective and Entertainment

If I am booking a Live Chart Reading, Recorded Astrology Reading, or Recorded Tarot Reading, I understand that these services are reflective and entertainment in nature. They are not predictions of specific future events, and I will not use them as a basis for medical, legal, or financial decisions.

6. Personal Responsibility

I accept full responsibility for my own physical, mental, emotional, financial, and relational wellbeing during and after sessions. I will use sound judgment in evaluating any guidance offered and in deciding whether and how to apply it.

7. Honest Disclosure

I will provide honest information during intake. I understand that the quality and safety of any service depends on the accuracy of what I share.

8. Assumption of Risk

I understand that reflective work, including therapy, coaching, and symbolic readings, can bring up emotional responses, memories, or insights that may be uncomfortable. I accept this as part of the experience and assume any associated risks.

9. Release of Liability

To the maximum extent permitted by law, and except as required by law for licensed clinical practice, I release Okairy Rodriguez and any associated business entity from any claims, liabilities, demands, damages, costs, and expenses arising from or related to my participation in any service or my application of any session content.

10. Emergency Protocol

I understand that if I experience a medical, psychiatric, or safety emergency, I will seek immediate help from qualified professionals or emergency services. Sessions are scheduled in advance and are not appropriate for emergency response.

11. Capacity to Agree

I am 18 years of age or older. I have the legal capacity to enter into this acknowledgment and waiver. I have read and understood its terms, and I am signing it of my own free will, not under duress.
