Client Acknowledgment, Informed Consent, and Waiver

1. Voluntary Participation

I am participating in this service voluntarily and of my own free will. I have read the description of the service I am booking, the Terms of Service, the Privacy Policy, and the Disclaimer, and I understand the nature of what I am booking.

2. Credentials Disclosure Acknowledgment

I acknowledge that Okairy Rodriguez is a Licensed Clinical Social Worker (LCSW), licensed in California (LCSW #62675) and Texas (LCSW #113455), and that she does not practice under her clinical license through this Site. I understand that the Services offered on this Site are non-clinical, are not psychotherapy, are not subject to clinical confidentiality or recordkeeping standards, and are not a substitute for therapy with a licensed provider in my state.

If I am seeking clinical psychotherapy or any licensed clinical service, I will contact a licensed provider in my jurisdiction.

3. Service Selected, Not Substituted

I understand the difference between Mentorship, Live Chart Reading, Recorded Astrology Reading, and Recorded Tarot Reading, as described in the Disclaimer. I am selecting the specific service that fits what I am looking for, and I understand that one service is not a substitute for another.

4. Telehealth / Video Delivery Consent

For any live session (Mentorship, Live Chart Reading), I consent to video delivery and acknowledge the following:

• The session will be conducted by video conference, ordinarily via Zoom.
• I will join from a private space where I cannot be overheard, with a stable internet connection and working video and audio.
• I understand video delivery has limitations compared to in-person work, including reduced ability to observe some non-verbal information and the possibility of technology failure.
• If technology fails, Okairy and I will follow the protocol in the Terms: attempt to reconnect within 10 minutes, then switch to phone, reschedule, or prorate as appropriate.
• While reasonable safeguards are used, no electronic communication is fully secure. I accept the limited but real risk inherent to video delivery.

5. Recording Consent

I understand that live sessions are not recorded by default. If recording is mutually agreed for note-taking or continuity, I will be informed before recording begins and may decline or withdraw consent at any time. I will not record any session without Okairy's prior written consent. I understand that recording without consent may result in immediate termination of the service without refund and may violate applicable law.

If I have booked a Recorded Astrology Reading or Recorded Tarot Reading, I understand the recording is created specifically for me, is delivered via private link, is for my personal use only, and may not be redistributed, posted publicly, or used commercially.

6. Not Medical, Not Psychiatric, Not Legal, Not Financial

I understand that no service offered through this Site is medical care, psychiatric care, medication management, legal counsel, or financial advice. For any concern outside the scope of the service I am booking, I will consult a qualified, licensed professional in that field.

7. Astrology and Tarot Are Reflective and Entertainment

If I am booking a Live Chart Reading, Recorded Astrology Reading, or Recorded Tarot Reading, I understand that these services are reflective and entertainment in nature. They are not predictions of specific future events, and I will not use them as a basis for medical, legal, or financial decisions.

8. Personal Responsibility

I accept full responsibility for my own physical, mental, emotional, financial, and relational wellbeing during and after sessions. I will use sound judgment in evaluating any guidance offered and in deciding whether and how to apply it.

9. Honest Disclosure

I will provide honest, accurate information during intake and during sessions. I understand that the quality of any service depends on the accuracy of what I share.

10. Assumption of Risk

I understand that reflective work, including coaching and symbolic readings, can surface emotional responses, memories, or insights that may be uncomfortable. I accept this as part of the experience and assume any associated risks.

11. Release of Liability

To the maximum extent permitted by law, and except for liability that cannot be waived under applicable state law, I release Okairy Rodriguez and any associated business entity from any claims, liabilities, demands, damages, costs, and expenses arising from or related to my participation in any service or my application of any session content.

12. Emergency Protocol

I understand that if I experience a medical, psychiatric, or safety emergency, I will seek immediate help by calling or texting 988, dialing 911, or going to my nearest emergency room. Sessions are scheduled in advance and are not appropriate for emergency response.

13. Cancellation, Refund, and Scheduling Policies

I have read and accept the cancellation, refund, and scheduling policies in the Terms of Service, including the 24-hour refund window, the 90-day credit policy after that window, the requirement to schedule live sessions within 14 days of payment with at least 3 days lead time, and the 24-hour rescheduling notice requirement.

14. Capacity to Agree

I am 18 years of age or older. I have the legal capacity to enter into this acknowledgment, informed consent, and waiver. I have read and understood its terms, and I am signing it of my own free will, not under duress.
